This worksheet gives Grade 4 students focused practice with one of the most useful comma rules: how to punctuate dates and addresses. Students learn to place a comma between the day and the year, between the city and the state, and — crucially — after the year or state when the sentence keeps going. That second comma is the one most students forget. The sheet moves from recognition to correction to original writing, so each section builds on the last. Start by reviewing the rule box at the top of the page, then work through the sections in order. Students who can write their own example sentences in Part D are applying the rule confidently.

Remember
- Use a comma between the day and the year in a date: She left on June 5, 2022.
- Use a comma between the city and the state: The bus came from Dallas, Texas.
- If the sentence continues after a date or state, add another comma: On May 1, 2021, school started.
